Observed both employee and customer restroom doors not self closing and left ajar at the time of inspection. Except where a toilet room is located outside a food establishment and does not open directly into the food establishment such as a toilet room that is provided by the management of a shopping mall, a toilet room located on the premises shall be completely enclosed and provided with a tight fitting and self-closing door. PIC stated they would put work order in to allow door to be self-closing. Corrective Action: Correct prior to next routine inspection.

PIC indicated that all team members have food handlers certificates but was not familiar with the term Certified Food Protection Manager (CFPM) , and stated they did not have CFPM. Certified Food Protection Manager (CFPM) is required to be on the premises during all hours of operation. Corrective Action: Correct prior to next routine inspection.
