Observed large sections of missing cove base in high moisture areas of the kitchen such that there are large (3-6 inch wide) openings into the wall/cove base. Discussed with person in charge to repair the hole and missing cove base. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. 6-201.11 - Core: Floors, Walls and Ceilings-Cleanability Addl Notes: Observed numerous sections of broken/missing tiles throughout such that the floor is difficult to maintain clean. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Discussed with person in charge to have the floor repaired.
