Small holes in wall panel near mop sink faucet handles. Caulking on top edge of hand wash station in men's restroom is cracked and separating from the wall. Women's restroom hand wash station shows beginnings of cracking also. Damaged coved base tiles in center empty area and near walk-in doors. Spacing around pipe near walk-in doors can be covered or filled.

Observed diced tomatoes, shredded cheddar cheese, and sour cream ranging from 45*F to 51*F in the reach-in of the make-line unit. Ambient temperature of the unit was 52.8*F using min/max thermometer. Person in charge (PIC) stated the tomatoes were placed in the unit at approximately 8:00 am from the walk-in and voluntarily discarded them - see embargo form. Time of inspection was approximately 12:45 pm. PIC stated the cheddar cheese and sour cream were placed in the unit from the walk-in less than 4 hours ago and relocated the items to the walk-in to rapid cool. Corrective Action: Corrected at time of inspection.

Observed downstream shutoff device installed on the mop sink downstream from an atmospheric vacuum breaker. PIC will remove the shutoff device from the mop sink. Discussed the importance of keeping shutoff device removed from the mop sink to ensure the AVB backflow preventer remains functional. Shutoff valves are not permitted downstream from an AVB backflow preventer. Corrective Action: Correct prior to re-inspection.
