Observed the storage shed out behind the main restaurant with the front door open and small gaps located around the windows. The shed is used to store dry goods and some produce including potatoes and onions. Also observed a sticky trap with one dead American cockroach stuck to it inside the shed. In addition, observed gaps in a door with outdoor access located near the restrooms with gaps in it as well. The establishment shall maintain all outer openings sealed to prevent the attraction and harborage of pests. Corrective Action: Correct prior to next routine inspection. *2nd consecutive violation

Corrective Action: Correct prior to next routine inspection. 6-201.11 - Core: Floors, Walls and Ceilings-Cleanability Addl Notes: Observed multiple broken floor tiles in the kitchen area and several sections of floor with diminished grout in between the floor tiles. Observed multiple Time/Temperature Control for Safety (TCS) food items including Chile relleno, cooked fish, raw shrimp, and raw beef located in a prep-top style refrigeration unit directly opposite from the grill registering internal temperatures ranging from 45°F-48°F. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. The perishable food items were still within proper Time/Temperature parameters. All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Corrective Action: Corrected at time of inspection.

Observed 2 prep-top style refrigeration units located along the prep-line registering ambient air temperatures between 43°F-50°F. Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below. All perishable food items located in the refrigeration units were moved to the walk-in refrigerator to rapidly cool. The establishment shall not use the refrigeration units for cold-holding food items until repairs are made. Corrective Action: Correct prior to re-inspection. *Reinspection will take place on, or before, Thursday August 15th.

At the time of inspection, establishment did not have any chlorine sanitizer test strips. The establishment only had pH test strips on site. Test strips are required for any sanitizers used with food contact surfaces. Provide sanitizer test strips prior to re-inspection.
