Inspector's notes
Observed a large crack/open seam that is forming in the tiles (grout seam) in the kitchen adjacent to the handwashing sink. It was observed that in some areas, the tiles appear to be shifting, with some of the tiles beginning to crack. The crevice that is forming is such that the floor in that area is no longer smooth or easily cleanable. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Observed ants crawling throughout the kitchen primarily underneath the fruit and vegetable rinsing sink and also crawling along the wall above the 3-compartment sink area. The kitchen needs to be treated by a licensed pest control applicator. Corrective Action: Correct prior to next routine inspection.

Please ensure that kitchen is regularly cleaned to minimize accumulation of food debris which may attract pests. Discussed with person in charge cleaning kitchen more frequently. Corrective Action: Corrected at time of inspection. 6-101.11 - Core: Surface Characteristics-Indoor Areas Addl Notes: Observed a missing tile on the floor next to the ice machine. The area with the missing tile is no longer smooth, durable and non-absorbent. Observed some penetrations in the tiles on the wall above the 3-compartment sink, which is allowing ants to move in and out of the wall. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.
