Observed employee finish sanitizing cook line and then removing gloves and proceed to put on new gloves to switch tasks to grill duties without washing hands. Discussed with employee to wash hands whenever they switch tasks and discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Hands must be washed at any time contamination occurs and when changing tasks. Employee properly washed hands at handwash sink at time of inspection. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Core-6-501.11, C: Repairing-Premises, Structures, Attachments, and Fixtures-Methods Ensure the following items are corrected as soon as possible: 1. Repair drain line for dipper well at soft serve ice cream machine. Ensure drain line remains rigid with a one inch air gap above floor sink. 2. Ensure escutcheon plate in back of soft serve ice cream machine is re-sealed to wall. 3. Chlorine sanitizer at dishwashing machine was not dispensing, however 3-compartment sink has quaternary ammonia.
