-- No paper towels available for hand drying in women's restroom at the time of inspection. Hand dryer sensor in men's restroom not working and there are no other means for hand drying. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. The PIC placed paper towels in dispenser in women's restroom and will place a work order to repair hand dryer. Corrective Action: Corrected at time of inspection.

The person in charge unsure when the lettuce and tomatoes were removed from cold holding and placed on the line. At the time of inspection, the lettuce and tomatoes were discarded. TPHC requires written procedures detailing process for TPHC and time-stamping for items actively held under TPHC. Corrective Action: Corrected at time of inspection.
