-- Observed customer handle box of pizza that was provided to them, notify an employee that they had received the incorrect item, and return the pizza back to the employee. The employee was then observed placing the pizza back into the warmer, where it appeared to be intended for service to another customer. Educated employee and person in charge: returned food may not be re-used or re-served. Once food has been in a customer's possession it is considered contaminated and a safety risk, meaning it must be discarded if returned. The pizza was voluntarily discarded at the time of inspection. Corrective Action: Corrected at time of inspection.

Corner panel missing from beverage storage shelf. Corner panel on same shelf farther down hall is coming off the edge. Coved base in back of storage area is peeling off the wall. Escutcheon covers for pipes near mop sink and pizza oven have moved away from the wall and should be re-adhered to the wall. One damaged coved base tile near walk-in door. Caulking at junction between mop sink basin and wall is missing. Caulking on junction of wall and hand wash station at front of store is beginning to separate. PRIORITY VIOLATION-5-202.13 - Priority: Backflow Prevention, Air Gap Addl Notes: No air gap provided to sanitizer basin drain line at 3 compartment sink. Drain line descends into the floor sink below the rim. Please shorten the drain line to provide a minimum required air gap between the end of the drain line and the rim of the floor sink (distance between end of drain line and rim of floor sink must be minimum of twice the diameter of the pipe or 1 inch whichever is greater. Refer to provided backflow prevention handout).
