Women's restroom hand wash station has separated from the wall and should be repositioned and caulked along the top edge that meets the wall. Men's restroom and hand wash station in rear of kitchen is just starting to crack and could be refreshed as well. Rubber coved base in various areas of kitchen is separating from the wall or has gaps between sections that expose the inner wall. Some of the areas include in the back corner near the mop sink, adjacent to the mop sink, below the 3 compartment sink, below the hand wash station in rear of kitchen. Cover missing from hole in wall behind ice machine (filled with plastic bag currently). ***2nd consecutive violation*** Corrective Action: Correct prior to next routine inspection.

Holes in wall behind ice machine filled with what appears to be plastic bags or paper towels and should be replaced with plastic or metal covers. Women's restroom sink is separating from the wall and should be repositioned and caulked along the top edge. Mop sink basin is beginning to erode and expose the aggregate below. Please resurface.
