2ND CONSECUTIVE VIOLATION Observed an accumulation of ice around the 2 walk-in freezer doors and a pipe for one of the condensers. Discussed removing the ice build-up and insulating the pipe as necessary to prevent damage to the unit and product inside of the freezer. Also discussed replacing the damaged gaskets around the doors. All equipment must be maintained in good repair at all times. Repair all equipment to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Employee left the production area to answer the door/phone buzzer two separate times; then came back into the production area; changed gloves and went to handle food items and clean equipment without washing hands. PIC stated that the employee would be properly trained on when to wash hands. Ensure that employees wash hands after engaging in activities that may contaminate hands and when changing tasks.
