Inspector's notes
Observed prepackaged salsa with TCS cut tomatoes 61-65*F, street corn with milk as ingredient 51*F, guacamole with TCS tomatoes as ingredient 61*F, queso with milk as ingredient 60-61*F on cooler case by deli department. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. PIC stated cooler would have been stocked over 4 hours prior and no temperatures had been taken within the past 4 hours. All TCS foods improperly cold held were discarded by PIC at time of inspection. After discussion with the person in charge, it has been determined that the root cause of the violation was a verification breakdown, which resulted in the violation. Corrective Action: Corrected at time of inspection.

Observed a crack in the mop sink by the produce department and in the mop sink of the back storage area. Physical facility must be maintained in good repair at all times. Repair physical facility prior to next routine inspection. PIC stated work order is in for repair/replacement of both mop sinks. Corrective Action: Correct prior to next routine inspection.

PRIORITY VIOLATION-5-202.11 (A), P: Approved System At the back mop sink by the loading dock, observed a T valve attached to the mop sink with a subsequent Y valve attached to the T valve. The T valve had a direct connection to the chemical dispenser. The Y Valve was attached to a chemical dispenser and the other side was an open hose. Y valves and T valves are not approved systems on the mop sink and were removed at time of inspection. Neither chemical dispenser was in use at time of inspection. Chemical dispensers must be removed from the mop sink when not in use. Discussed approved systems and sent handout for approved systems via email. Correct By: Corrected At Time Of Inspection
