SECOND CONSECUTIVE. Observed boxes of toothpicks and gloves sitting in the basin of the handwashing sink in the corner nearest the prep tables. Facilities must be maintained in a condition that promotes handwashing. PIC relocated all items blocking the handwash sink at the time of inspection. Discussed maintaining soap, paper towels, and a trash basin near ALL the handwashing sinks. Observed 0 PPM of quat dispensing from the three-compartment sink. The manager stated they had previously had issues with the dispenser. Staff will use the nearby working mechanical dishwasher to wash, rinse, and sanitize all dishes until the automatic dispenser is serviced and capable of dispensing quat within the recommended range (150-400 PPM). Discussed the importance of sanitizing and keeping equipment in good working order. Corrective Action: Corrected at time of inspection.

Observed tomato salsa at 43-45*F in deep containers with lids on in the walk-in cooler. According to PIC, the salsa was made the day prior and placed in deep, greater than 4" containers. Cooked TCS foods must be cooled from 135°F to 70°F within 2 hours, and from 70°F to 41°F within 4 hours for a total of 6 hours for cooling. All TCS foods improperly cooled were discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Observed tomato salsa in a deep, greater than 4", tightly covered container in the cooling process at 61*F. Salsa had begun the cooling process within the past 6 hours according to the PIC. TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. PIC transferred time/temperature as a control (TCS) tomato salsa to shallow containers less than 4" and added ice as an ingredient for rapid cooling. Corrective Action: Corrected at time of inspection.
