PRIORITY VIOLATION-5-202.11 (A), P: Approved System>>>Observed chemical dispenser water intake hose attached to a mop sink faucet equipped with a atmospheric vacuum breaker (AVB) via a y-valve connecter; the other end of the y-valve was connected to a hose. Person in charge was informed that AVB back flow devices are not designed for applications where they could be subjected to continuous water pressure, and are not testable for maintenance such as when they are connected directly to a chemical dispenser/y/valves; this could lead to the failure of the AVB over time. Establishment must ensure there is a independent water line for the AVB and that the AVB is equipped with full 1 inch air gap connectors internally or they may install a high hazard back flow device (such as a reduce pressure zone backflow device) between the water line and the chemical dispenser. Establishment must ensure that all sinks equipped with a AVB do not have a y-valve attached downstream. Re-inspection 10/20/23. Guidance provided over email.
