The floor drain at behind the front counter and the mop sink have several pipes draining into them without a 1 -inch air gap. The pipes are below the rim line. Discussed with person in charge (PIC) that they will need to be lifted an inch above the rim line to reduce risk of contamination. All plumbing must be maintained in good repair at all times. Corrective Action: Correct prior to next routine inspection.

Observed built up oil, food debris, and pooled water on the ground throughout the back kitchen area. Person in charge (PIC) and employees did not know where the pooled water in front of the 3-compartment sink had come from. Inspector could not find any leaks at time of inspection. Physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris. Must clean physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.
