A Facilities inspection was conducted. 1) Discussed with Person in Charge (PIC) the mop sink with the Atmospheric Vaccuum Breaker (AVB) shall not be directly connected to the chemical dispenser when not in use to the faucet with a Y-valve. Please have a separate water line installed upstream from the AVB for the chemical dispenser in order toallow the AVB to operate without being under constant pressure. The faucet with the AVB should be able to be used independently of the chemical dispenser. This will prevent chemicals from being syphoned into the clean water supply. Also discussed with PIC the installation/use of a Y-valve on a faucet with or without an AVB is prohibited, because the system is not designed, constructed and installed according to law. 2) PIC demonstrated the high temperature dishwasher thermometer. Dishwasher did not reach an internal temperature of 160*F after running the equipment 4 times. PIC placed a "Do not use" sign on the unit stating to utilize the three compartment sink.

Inspector's notes

Observed at mop sink in the maintenance room a not-in-use chemical dispenser that is directly connected via a hose to a faucet with an AVB. Discussed with Person in Charge (PIC) to add a separate water line upstream from the AVB for the chemical dispenser in order to allow the AVB to operate without being under constant pressure. The faucet with the AVB should be able to be used independently of the chemical dispenser. This will prevent chemicals from being syphoned into the clean water supply. PIC disconnected chemical dispenser at time of inspection. Corrective Action: Corrected at time of inspection. 5-205.15 (B) - Core: System Maintained in Good Repair; Maintained Addl Notes: Observed at the dishwasher drain line under the three compartment sink to be resting inside of the floor sink. Discussed with PIC drain lines must have at least a 1-inch air gap above the flood rim of floor sink. Cut lines/equip with shorter extensions/hoist lines so that there is at least a 1-inch air gap between flood rim and each drain line to avoid back syphoning of contaminates. Corrective Action: Correct prior to next routine inspection.
