Construction complete and approved.Backflow test will take place this afternoon and reports will be texted over.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. Pre-final walk through with the superintendent. The following items were discussed for follow up before final inspection:1). Beverage machines will be in place and will be outfitted with dual check valves that meet plumbing standard ASSE 1022.2). Caulk the gap between the 6 inch pipe and the wall used for soda bundle under the front beverage station, not sure what to do with the tile work at the other station, also maybe some caulk in the corners.3). Cleanout covers will be installed throughout, make sure they are installed in also the hard to spot locations under counters.4). For service bar top and passthrough windows, please caulk any joints that are open, also caulk stainless and wall joints where open, base where open between material like at entrance of front service area, and finish out edges of the mop sink curb wit additional trim.5). Caulk/ seal, stainless joints where open, like under hood and between hoods, also gap in stainless at mop sink.6). All fixtures will be caulked/ siliconed at the top wall junctures.7). Close the gap between the hand wash sinks counters in the restroom and the wall.8). In the restroom, where the schluter stands open from the floor, fill in and provide cleanable area.9). The combi-oven will receive a reduced pressure zone valve upstream, both this valve and soda valve will require testing and certification before final.10). Make sure the spray arm assembly at the dish sink does not reach below the flood rim, adjust.11). The ice machine will be plumbed in with rigid drain lines.12). At the front soda station, line of the backflow valve hub drain line with the other drain line and provide a 1 inch air gap.13). In the walk-in units, fill in the sprinkler penetrations.14). Make sure the chemical dispenser installed at the mop sink has an internal air gap.15). Back door will be self closing, air curtain will me micro switch activated and aim the air 2-3 % outward.16). FRP trims and corner guards will be finished up.At final inspection, make sure the list is complete, al equipment is in place, refrigerators and walk-in units are at temperature and the you have hot water at all sinks.No County legal action will result from this inspection.Printed on site.

Stopped by the site and checked records. Plumbing inspection was done in August. Finishes in the back of the house are in except for the ceiling. Walk-in u nots are in place. Hood is near completion. Plumbing inspection conducted with site superintendent, Jose.At the time of this inspection, this project was about 20% complete. Inspection report was emailed to Jose.Prior to final:1. Provide ASSE 1013 RP upstream of carbonators, combi oven and rethermalizer as discussed. 2. Provide ASSE 1022 dual check valve for tea and coffee.3. Provide rigid drain lines for all equipment. Must be rigid with 1 inch air gap above floor sinks.4. All paint in restrooms and FOH must be semi or high gloss as discussed.Confirmed air curtain, additional line at mop sink, and other items noted on Stipulation Letter.Please continue with construction according to plans, stipulation letter, and inspections.
