Inspector's notes
Deli style ham, turkey, roast beef, salami, and sliced cheese with internal temperatures between 43 degrees F and 46 degrees F cold holding in meat cooler near the entrance to the kitchen. Ambient temperature of the cooler was 44.4 degrees F using a min/max thermometer. PIC stated that bottom cooler door was broken and a work order was placed for the unit. All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. TCS items that remained in the cooler (deli meats and cheeses) were placed under time as a control until the unit is repaired. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. Corrective Action: Corrected at time of inspection.

Deli style ham, turkey, roast beef, salami, and sliced cheese with internal temperatures between 43 degrees F and 46 degrees F cold holding in meat cooler near the entrance to the kitchen. Ambient temperature of the cooler was 44.4 degrees F using a min/max thermometer. PIC stated that a work order was placed for the cooler. Establishment has enough cooler capacities to operate safely until cooler is repaired. All equipment must be maintained in good repair at all times. Repair all equipment to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Final Construction Inspection - E&D Class 3This inspection was conducted with a contractor on site.A PDF copy of this inspection report was emailed to Aaron S. at the time of this inspection.Correct the following items prior to your first routine inspection;1. The hot and cold water valves on the mop sink faucet are reversed. Repair this faucet to have hot water on the left valve and cold water on the right valve.2. Provide a stainless steel, ASSE 1022 backflow prevention device with a screen on the water supply to the tea machine. Install this downstream from the inline shutoff valve to this machine so that the tea machine is isolated from the all other fixtures connected to the same water supply line.3. Provide another ASSE 1022 backflow prevention device with a screen for the water supply to the ice machine above the soda fountain. Install this downstream from the filter to the ice machine so that the ice machine is isolated form all other fixtures connected to the same water supply line.4. Provide minimum 1 in. air-gaps between the flood rim of the mop sink basin and the ends of the three drain pipes that empty into it.5. Re-caulk and seal the spaces between the mop sink basin and the surrounding walls.If violations are not corrected by established timeframes, legal action may result.This establishment has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Provide a dedicated water line at the mop sink for chemical dispensers. Mop sink faucets cannot be used to supply water to chemical dispensers.2. Provide a certified Double Check Valve Assembly for the water supply line to the Proofer oven.3. Provide a certified, RPZ backflow prevention device for the water supply line to the soda fountain carbonator. All piping and fittings on the downstream side of this device must be plastic or stainless steel, no copper or brass.4. Provide a sneeze guard with side panels for all exposed food areas along the front service counter.5. Provide a cove base finish at the floor/cabinet junctures inside the cabinets beneath the self-service soda fountain.6. Seal all the spaces between the walk-in units and the surrounding walls.7.
