Inspector's notes

Observed back door to establishment wide open at the time of inspection. The establishment shall maintain all doors to the outside closed to prevent the introduction of pests into the building. In addition, observed fruit flies inside the establishment near the mop sink and 3-compartment sink. Person in charge closed the door at the time of inspection. Corrective Action: Corrected at time of inspection.

Inspector's notes

Observed an unattended hose with attached spray nozzle connected to the mop sink faucet with the water in the on position. Discussed with the manager maintaining the spray nozzle disconnected and water turned off when not in use to prevent the failure of the atmospheric vacuum breaker backflow prevention device. Person in charge disconnected the spray nozzle and turned off the water at the time of inspection. Corrective Action: Corrected at time of inspection.

Inspector's notes

Observed multiple deficiencies with the plumbing inside the establishment including a small leak at the mop sink faucet and the condensate line for the ice machine in disrepair leading to water leaking onto the ground of the back room. The establishment shall maintain all plumbing on the premises in good repair. Corrective Action: Correct prior to next routine inspection.

Inspector's notes

No shelving in the walk-in cooler so all food/beverages are stored on the floor. PIC stated that shelving is on back order and will be put up as soon as it comes in. PIC ensured that food was off the ground and in containers. Food must be stored at least 6 inches off the ground at all times to prevent contamination from the premises. Corrective Action: Correct prior to next routine inspection.
