Observed the meat band saw covered with a rug, boxes, and small white dry particles and all prepping tables covered with brown/ yellow/white powders. Discussed with person in charge (PIC) all food-contact surfaces must be cleaned to sight and touch. PIC started to wash the prep table and relocate rug and boxes. Discussed once meat band saw is cleaned to protect with food grade bags or food grade plastic wrap. Corrective Action: Corrected at time of inspection.

Observed several dead German cockroaches and dried corn and other dried grains throughout the back of the facility. Discussed with PIC establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must remove dead pests to prevent the attraction of more pests. Also discussed to clean up all foods and grains off the ground to prevent the attraction of pests. Corrective Action: Correct prior to next routine inspection.

Personal bulk foods must be maintained at home and not co-mingling with foods/ ingredients for consumers.

Observed the hand wash sink and the three compartment sink storing empty boxes in the back of the facility. Discussed with the Person in Charge (PIC) the hand wash sink shall be maintained as accessible at all times. PIC relocated the boxes, and the sink was made accessible at time of inspection. PIC will ensure establishment discontinues use of handwash sink for all activities that are not for handwashing. Corrective Action: Corrected at time of inspection.
