Juices are not pasteurized, and as such require a consumer advisory posted at point of sale. Discussed consumer advisory requirements and provided guidance document for juicing. Must provide approved consumer advisory at all points of sale prior to re-inspection. Corrective Action: Correct prior to re-inspection. **Re-inspection scheduled for 11/6/25. Final construction inspection.Construction is complete and approved.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Unscheduled equipment inspection.Construction is about 75% complete.Address the following items:(1) Have the sneeze guard installed.(2) Provide a splash guard between the prep sink and the 3 comp sink.(3) Provide a separate waterline at the mop sink for chemical dispenser connections.(4) Provide a swing arm for the 3 compartment sink. Swing arm must reach all 3 compartments.(5) Provide metal shelving over the 3 compartment sink (6) Provide a split-front, commercial toilet seat in the restroom.(7) Provide a rigid drain line (PVC) for the ice machine, maintain a 1 inch air gap above the floor sink.(8) Provide T-grig/ceiling tile around the roof access ladder.(9) The exposed insulation at the roof ladder access must be scrimmed or removed.Please continue with construction according to plans, stipulation letter, and inspections. For final approval, have all work completed, have all items on site, including small wares, have hot water available to all sinks, have all refrigeration maintaining proper temperatures, and have all local municipality approvals.Please call (602) 527-0893 for a final inspection at 100% completion of project or if an additional onsite walk-through is needed.
