Observed TCS meat and vegetables in the walk-in cooler 45*F-49*F at the time of inspection. Per inspector min/max thermometer, the internal temperature of the unit read 49.3*F and per the digital thermometer on walk-in unit, it was 50*F at the time of inspection. Per PIC, they had checked temperatures of the unit less than 4 hours prior to the time of inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. TCS items greater than 41*F were placed in alternative functioning units at the time of inspection. Corrected at the time of inspection.

E&D class 4, 10+ seating.Establishment is currently using chlorine sanitizer that, per manufacturer's instructions, may be up to 200ppm chlorine (dish machine). If you switch chemical companies or products in the future, ensure you follow the manufacturer's instructions for the proper ppm range for food contact surfaces.Correct the following items prior to your routine inspection:1) Seal all gaps in ceiling around penetrations such as fire riser, water heaters, etc. so that there are no gaps. If foam is used, trim flat and seal.2) Seal around electrical line in walk-in cooler.3) Front service refrigerator not functional at time of inspection. Per operator, refrigerator is only to be used for glassware or other non-food or non-TCS items. Refrigerator is under warranty and will be serviced. Establishment has adequate refrigeration otherwise.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Original layout received and reviewed had a hand sink visible from cookline.Correct the following items:1) Provide a hand sink visible from cookline. Handwashing sinks must be centrally located, visible, and directly accessible within 25-feet of all food/beverage preparation, food/beverage dispensing and warewashing, wait stations, and similar workstation areas. Barriers must not physically and/or operationally obstruct the hand washing facility. Metal splashguards must be installed between the hand sink and food/ beverage/ utensil related areas within 24-inches of the hand sink.
