Suite is not open to the public.***Discussed applying for a minor remodel permit if Baxter oven will be connected to a water line (backflow device ASSE 1015 required), and if back corner room near water heater will be converted to a prep area. Provided remodel link in email sent to Mark B.Correct items noted prior to the next routine inspection.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select Pay a Fee at esd.maricopa.gov. OC Re-inspection, Bakery. Conducted with owner Mark B. Establishment was not open and operating to the public. Establishment will be wholesaling only, and suite will not be open to the public.Gas will not be used at this time. Owner will only use electric ovens.Correct the following items prior to reinspection:1) Remove the flex portion of the drain line connected in between the newly installed hand sink. 2) Replace the 3-compartment sink drain line. Water was observed leaking from the drain lines and observed duct tape around one of the pipes. 3) Provide labels for all packages: ingredients in descending order, contact information, known allergens, and address of establishment where the product is made. 4) Provide missing/burnt out lights throughout the establishment, walk-in cooler and under the hood.5) Finish removing the unwanted/misc. items throughout the establishment. 6) Completely fill in the void to the left side of the floor sink. Epoxy, tile or siliconized grout may be used. 7) Provide a new door sweep underneath the front door, and ensure there are no gaps on the sides of the front door. 8) Detail clean throughout the establishment. Please contact the Department to schedule a follow-up inspection at 602.506.6980 once violations have been corrected. Please be advised that the New Business Permit Application fee includes one initial inspection and one follow-up inspection. Additional fees will be assessed for any additional follow-up plan review inspections.No County legal action will result from this inspection.
