Ensure that the end of the faucet is always at least 1 inch above the flood rim level. Corrective Action: Correct prior to re-inspection (within 3 days). Send picture to confirm correction, gave manager my card. **Visited site on 7-31-25. Faucet is still in the same state and not corrected. Gave business card to staff on site. Contact when faucet has been corrected. Will check again next week. Visited site on 8-11-25. Facility has repaired the air gap at the 3 compartment sink. Corrective Action: Corrected at time of inspection.

Hot handle to be on the left.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Route funnel/drain to the floor sink.(2) Flash off the gaps around the walk-in door.(3) Flash of the walk-in where it meets the walls near the back door.(4) Finish painting and ceiling grid the above the walk-in.(5) Finish installing the hub drain for the blender rinse sink.(6) Provide a split-front, commercial toilet seat in the restroom.(7) Finish all caulking and trim detail work.Please continue with construction according to plans, stipulation letter, and inspections.
