Final construction inspection. Construction complete and approved.Superintendent will follow up on the item below before starting up:1). Faucet will be installed on the current faucet.2). Grease tray lid is ordered and will be installed today.3). Wheels will be installed on the commercial chest freezer.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. If you select payment via US Mail. First inspection walk through. Painter is on site but superintendent is not. Store does not have power. Walls are closed up and about finished out. Discussed project over the phone with the superintendent. The following items will be addressed or are in the works:1). Back door and restroom door will be self closing.2). Three compartment sink and prep sink will be located to the west of the mop sink. 3). If the three compartment sink does not have any drain boards then install racking above the sink for the air drying of washed dishes.4). Normally T&P and water heater drain pan are led to daylight or mop sink. Check with City of connection to a vent stack with trap seal is approved.5). Front of the house, back of the house and restroom will receive coved base.6). Escutcheons and clean out covers will be installed where needed.7). An additional water line was not installed at the mop sink for possible future chemical dispensers. If needed water will need to be drawn from below the three compartment sink.8). Water line to coffee machine has been omitted.9). Oven will not be connected to any water supply.10). Walls above FRP and wooden panel at back door will be finished out with a washable paint. Caulk any open joints.11). Hand wash sink on south east side may waste indirect, make sure to bring the drain line to the wall, then down and then forward again and properly brace.12). If you deviate from the originally specified sinks, make sure the new sinks are NSF approved.13). Required vertical sneeze guard height requirement is 60 inches.
