To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Priority Foundation-5-205.13, Pf: Scheduling Inspection and Service for a Water System Device; 1. The RPZ backflow prevention device for the water supply to the soda fountain carbonator must be tested and certified. Also, the copper and brass piping ad fittings on the downstream side of this device must be removed. All piping and fittings on the downstream side of this backflow prevention device must be plastic or stainless steel, no copper or brass. Core-5-205.15 (B), C: System Maintained in Good Repair; Maintained; 1. The hot and cold water valves on the food prep sink are reversed. Provide hot water on the left valve and cold water on the right valve. Core-5-203.13, C: Service Sink; 1. The old, deteriorated mop sink must be removed. 2. Provide a smooth, non-absorbent, easily cleanable wall surface behind the new mop sink where there is currently a stucco finish. Correct By: Correct Prior To Reinspection
PRIORITY VIOLATION-5-402.11, P: Backflow Prevention; 1. The drain pipe for the food prep sink is currently directly connected to the septic system. Provide a separate drain pipe that empties into the floor sink with a minimum 1 in. air-gap between the end of the drain pipe and the flood rim of the floor sink.
