To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. First inspection for the Domino's in Tolleson. Walk through with the superintendent. The following items were discussed:1). Floor sink for the walk in cooler has been omitted and the condensate will be run to the mop sink, see if you can run as much of the condensate line in the wall. Check line for leaks before closing up.2). Water heater tank will be in the restroom as well as the washer/ dryer combination. Try to space the water heater so you can clean around it or mount on a stand.3). Hot water loop will be run to make sure the front hand wash sink has hot water quickly.4). dedicated water line will be installed at the mop sink for possible future chemical dispensers.Store just finished up underground. No County legal action will result from this inspection.E-mailed report.
