Observed an employee plating a ready-to-eat cooked burger at the time of inspection. Observed employee guiding the RTE food item with their bare hand and touch the burger patty with their bare hand in the process. Discussed the importance of preventing bare hand contact with ready-to-eat (RTE) foods with the person-in-charge (PIC). Bare hand contact is not approved with ready-to-eat (RTE) food items. RTE food that was contacted by bare hands was discarded by PIC at time of inspection. Corrective Action: Corrected at time of inspection.

Observed cooked chicken with 8/14 date marking and TCS pizza sauce with 8/7 date mark. PIC stated both the chicken was cooked on and pizza sauce made on 8/19. PIC was instructed to properly date mark TCS foods at the time of inspection. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). Observed rodent droppings at the time of inspection. PIC has a licensed pest control company with invoices for treatment. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. PIC was instructed to clean up droppings. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection on 9/1/25.

Also, all fixed equipment shall be sealed, and any exposed wood shall coated with either high gloss or semi-gloss paint so it is smooth and easily cleanable.
