Final construction inspection for the Retail Grocery permit. Walk through with the superintendent. Construction complete and approved.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. If you select payment via US Mail. For further instructions on mail in payments please refer to the printed invoice.Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. No operating permit is valid, until the permit fee is received by the Department. Pre-final walk through for the Retail Grocery permit. Store is 95% completion. The following items were noted for follow up before final inspection:1). In the restrooms the hand sink counters will be siliconed in.2). Please touch up grout at:a). men's room last stall at cleanout cover.b). Door frame in janitors closet hall way.c). In ice room, that whole room should have been grouted to the wall before installing base.3). North side water manifold room, provide escutcheons for the drain line going through the wall.4). At the rinse sink in the kitchen:a). Lower the backflow valve or make removable for testing.b). Install a cleanout cover.5). Caulk the side of the hood in at the back wall juncture.6). Please silicone the door frame from kitchen to back room at the floor juncture where open.7). Plumbing, water supply and drainage of the ice machine needs to be finishes.8). Install a micro switch for the fly fan at the back door.9). Silicone the wall penetrations of the walk-in cooler condensate line, both sides, walk-in and water heater room. 10). Increase the water flow at the back room hand wash sink.11). Under the soda counter, provide:a). Drain lines for the drip pans, there are two- at slushy counter.b). Install another backflow valve for the water supply to the to be installed spray hose with nozzle. If you ca you may also tie into the existing backflow for the other spray hose.c). Have the valve tested,12). All walk-in sprinkler will need top be filled in to seal and restore R value.13). At the spray hose under the slushy machines the base is standing open.14). One of the backflow valves in the water heater rooms is venting water, please address.Next inspection will be final inspection. First walk through with the superintendent for the Retail Grocery Permit. Copper is complete and water lines are in place for all fixtures. Filtered water lines and RO lines are not all in yet. The following items were discussed:1). Super will check on the location of the floor sink to the side of the north side coffee counter. Page P201 detail 7 shows it not tp be in the corner.2). Water line for the ice machine will be surface mount.3). Page P512 detail A shows a spray hose also under the soda station. When installed please make sure to install a Reduced Pressure Zone on the water supply just like the other two locations.4). Page P202 detail A shows two hot water lines in the wall at the soft serve machine and rinse sink. Washer box was observed but not sure if we are missing a water feed. Please check.5). Dual check valve that meet plumbing standard ASSE 1022 will be installed for all non carbonated beverage machines.6). Reduced Pressure Zone backflow valves will be installed for:a). Pitcher rinser at rinse sink.b). Non-filtered water to oven.c). Upstream of RO system.d). Upstream of multi-plex.e). Upstream of the hose down cart.f). Upstream of the spray down hoses at the beverage counters - 3 in total.7). Dishwasher drain will transition into a rigid line under the three compartment sink.Walls are closed up and crew is working on installing the FRP.Next inspection is after sinks are set.No County legal action will result from this inspection.E-mailed report to superintendent.
