Final inspection walk-through. Construction complete and approved.This establishment/operation has been evaluated and qualifies for the issuance of the permit. To complete the permitting process, meaning issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Note that fees paid up to this inspection approval did not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Checks and money orders are also accepted via US Mail. If you select payment via US Mail. Pre-final walk through with superintendent. The following items will be addressed before the final inspection:1). Grease trap lid and clean out lid will be installed.2). The wood under the front counter will be caulked and wood painted- side cabinet only.3). Sinks will be caulked at the top wall juncture.4). Please caulk the top of the eliason door frame.5). Please install escutcheon plates where missing, one in the bathroom.6). Sneeze guard and return will be installed on the front counter. Please provide 60 inches from FF on vertical glass.7). Caulk the mop sink at the wall and floor junctures.8). Please replace the drain grate on the front floor sink so the grate is flush with the floor sink.9). Dipper well will be installed in the front. Drain line will have air gap and will be properly braced.At time of final inspection provide hot water.No County legal action will result from this inspection Printed on site

Back door has closer and weather stripping and door sweep.6).
