Final Construction inspection conducted with owner Alex B. Construction completed and approved. Inspection report was emailed to owner Alex B.***Michelada drink mix containing Clamato juice must be date marked for 7 days, and must be refrigerated after opening until the PH and water activity for the Clamato juice is provided and approved by the Department. ***Roll up door must not open without approved air curtains. Roll up doors must remain permanately closed. Contact the Department if air curtains will be installed. Correct the following items prior to routine inspection:1) Adjust the counter weights on the mop sink faucet and the 3-comp pre-rinse sprayer so the faucet does not go below the flood rim.2) Provide a 3/8” cove base throughout the kitchen, restroom and mop sink area.3) Finish applying a semi gloss paint to the walls in the mop sink area. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Conducted scheduled onsite inspection with Jocelyn B. Inspection report emailed to Alex B. Correct the following items prior to final inspection:1) Provide a floor sink to provide an indirect drain line for the 3-compartment sink. Provide a 1-inch air gap above the floor sink.2) Provide a 50-gallon hot water heater. Current 40-gallon hot water heater isn’t approved.3) Provide either a direct drain line for the hand sink or drain into the floor sink. Ensure hand sink has a separate line from the 3-compartment sink.4) Provide a new mop sink basin. Mop sink may be a floor mounted basin or a free-standing mop sink. Provide a standard faucet with hot and cold water. Provide a hose bibb backflow preventer for the mop sink faucet. 5) Relocate the furthest equipment from the hand sink so that it is within 25-feet or provide an additional hand sink.6) Refrigerators must be NSF approved with a stainless front. Retail refrigerators are not approved for repackaging and cooling of potentially hazardous products. Provide a stainless-steel fridge for the Michelada drink mix.7) Repair the pre-rinse sprayer at the 3-compartment sink so that it does not sit below the flood rim. Spring appears to be broken.8) Switch the hot and cold water lines at the hand sink next to the 3-compartment sink. 9) Roll up garage door must either have air curtains on the outside equipped with a micro switch or must be completely welded shut. Seal in all areas of the roll up door that shine light through. 10) Provide FRP starting from the bottom of the tile base all the way behind the 3-compartment sink and 12 inches past the drying racks or pre-rinse faucet. Provide FRP behind the mop sink.11) Provide additional information for the Michelada drink mix. Separate email sent to owner.12) Provide a semi or high gloss paint light in color throughout the facility. Walls must be easily cleanable. 13) Provide lighting in the mop sink area. 14) Provide a 3/8” siliconized cove base to the wall and floor juncture throughout the kitchen, restroom and mop sink area.15) Provide a U-shaped toilet seat.16) Provide a self-closer to the restroom door. 17) Provide door sweeps to both exterior doors.18) Repair/replace any broken floor tiles within the establishment.
