Observed zip lock bag of crab sticks on prep table at 50*F and container of spicy crab on prep table at 54*F. As per the person in charge, items were out to prepare food items for sale. Discussed time as control with staff and items were embargoed and discarded. Advised staff that food items must remain at 41*F or lower during preparation and storage,. Corrective Action: Corrected at time of inspection.

To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, by or phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit.
