Observed soap dispenser at main handwash sink and soap dispenser near prep sink not functioning. PIC was unsure if/when maintenance was scheduled on soap dispensers. Employees currently use hand sink in bathroom to complete handwashes. Handwash sinks must be supplied with hot water of at least 100°F, soap and an approved hand drying provision at all times. Facilities must be maintained in a condition that promotes handwashing. Corrective Action: Correct prior to re-inspection. Reinspection will occur on 4/10/26

Final Construction inspection conducted with site superintendent. Construction completed and approved. Establishment plans to open by the end of May 2024. Hot water temperature at hand washing sinks were at least 100 degrees F and at least 110 degrees F at all other sinks. Ambient temperature of reach-in refrigerators was 41 degrees F or below.Complete the remaining items by the next routine inspection:1. Replace missing ceiling tiles near mop sink.2. Seal pipe penetrations in ceiling tiles above hot water heater.3. Seal corner of FRP on wall to right of restroom door.4. Seal around restroom light switch cover.5. A certified food manager is required at time of approval. If not already obtained, please obtain your certified food manager certificate from an ANSI approved testing company. All other food handlers must obtain ANSI approved food handler certificates. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Plumbing inspection conducted with site superintendent. At the time of this inspection, this project was about 25% complete. Floor was open and trenched with plumbing exposed with wall stub-outs for handwashing sink, 3-compartment sink, prep sink, floor sinks, and mop sink. Site superintendent stated the existing water heater will be replaced with a new 50-gallon water heater. Restroom is existing but being remodeled. No equipment has been installed. Discussed floor finishes, wall finishes, ceiling finishes, and equipment placement. Discussed separate water line for chemical dispensers at 3-compartment sink and mop sink. An angle-stop valve is acceptable for the chemical dispenser at the 3-compartment sink as long as the chemical dispenser has an air gap. A separate water line will be required for any chemical dispenser connected at the mop sink. Ensure 3-compartment sink, prep sink, and drain line from water heater are indirectly drained to the nearby floor sink with a one inch air gap. Inspection report was emailed to Brandon Huffman.Please continue with construction according to plans, stipulation letter, and inspections. Please call (602) 402-3147 or the inspection request line 602-506-6824 for an equipment/construction inspection between 50 - 75% completion of project or if an additional onsite walk-through is needed. No county legal action will result from the inspection.
