Per Person in Charge (PIC) the salad mix was made, portioned and placed in prep table more than 4 hours prior to inspection. Discussion with PIC it has been determined that the root cause of the violation was training staff on the policy to have temperature control for safety (TCS) items at a temperature of 41*F before being put into a prep table, which resulted in the violation. Embargoed salad mix and PIC voluntarily discarded the salad mix. Correct By: Corrected At Time Of Inspection
