OC re-inspection conducted with Sean.Inspection report emailed to Sean M.All items have been addressed.Of Note:Hand sink faucet closest to the entrance was working, but flow of water became limited. This may be due to deposits building up in the aerator after use. PIC tried to remove aerator at time of inspection to clean, but unit was too tight. PIC will have hand sink looked at again.No county legal action will result from the inspection. Additional fees will be assessed for any additional follow-up plan review inspections.This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Core-6-501.11, C: Repairing-Premises, Structures, Attachments, and Fixtures-Methods Address the following items prior to re-inspection: 1. Restore hot water to the hand sink at the entry way. 2. Clean the ice machine dispenser inside the unit. 3. Provide rigid line for the flex tubing of the water filter to ensure it maintains air gap with trough. 4. Remove unused water line underneath the prep sink. 5. Provide physical air gap for the chemical dispensers as discussed (air gap photo emailed). 6. Have the walk-in freezer serviced, ice is built up on the condenser coil. 7. Reseal the dish table where black organic growth is forming and hand sink backsplash. 8. Clean off the mechanical warewash and have the hood vent serviced (debris are falling onto the unit from hood vent). Correct By: Correct Prior To Reinspection
