Discussed with PIC fix/repair/replace the cause of the leak in the walk-in freezer creating icicles, remove the icicles and to not store food under the frozen liquid until it is fixed. No contamination observed. Also observed the walk-in refrigerator with a bucket of water underneath the fan. Per PIC the fan does leak into the bucket. No contamination observed and discussed with PIC to not store food under the fan that is leaking until it is fixed. Repair all equipment to a state of good repair prior to next routine inspection. Observed grilled chicken with an internal temperature of 155*F in the top of the pizza prep table. Also observed cooked sausage at 45*F right next to it. Per person in charge (PIC) the items were cooked/opened an hour prior to inspection. Discussed with PIC TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. Discussed the pizza/sandwich prep tables are not adequate refrigeration for cooling. PIC relocated the cooked chicken and cooked sausage to adequate refrigeration such as the walk-in to properly cool at time of inspection. Corrective Action: Corrected at time of inspection.

Observed the facility to not have a working probing thermometer to take internal temperatures of cooked meats. The thermometer onsite was unable to be calibrated and was deemed nonfunctioning. Entire process should take approximately 20 seconds for full and proper handwashing to prevent spread of pathogens and contamination. 2) hand washing shall occur before putting on gloves, in between taking off gloves and putting on new gloves, and gloves shall be worn before touching ready to eat RTE foods and garnishes. 3) Soda nozzles shall not be soaking in water when not in use. All soda guns, soda nozzoles and soda gaskets shall be cleaned by soap, rinse, sanitize and airdry.

Observed the chlorine sanitizer below 50ppm at time of inspection. Employee added more chlorine and observed the chlorine sanitizer to be at 50ppm at time of inspection. Discussed with person in charge (PIC) the importance of proper sanitizer levels and proper monitoring of sanitizer levels. Not all items have labels and the ones that did only stated what was in the container. Observed no date marking system in place. Per PIC some of the TCS items were made/prepped/package opened were 1 to 2 days prior to inspection. Observed several TCS items on the bottom part of the pizza prep table with an internal temperature of 47 to 50*F. The ambient air temperature of the bottom of the unit was 48*F measured with a min/max thermometer. Per PIC the items were placed in the unit 1 hour prior to inspection. Discussed with PIC except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. PIC All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. The TCS items relocated were burgers, grilled chicken, chicken burgers and sliced tomato. The non-TCS items that were kept in the bottom of the refrigerator was sweet chili sauce, prepackaged blue cheese dressing and ranch. Also observed the top part of the prep table holding TCS foods at 41*F measured with a probing thermometer and the top part of the unit holding at 41*F measured with a min/max thermometer. The TCS items kept in the top part of the unit was cooked sausage, pepperoni, pizza sauce and mozzarella cheese. Discussed with PIC to monitor the TCS items in the top part of the pizza prep table to ensure the temperature continues to hold at 41*F or less. Corrective Action: Corrected at time of inspection.

Observed in the bottom of the pizza prep table to be holding a liquid. Soaked towels are in the unit under the food containers. No contamination was observed at time of inspection. Observed frozen liquid icicles buildup on the fan in the walk-in freezer. Observed food stored underneath the frozen liquid icicles. Due to contamination observed in an open bag of mozzarella sticks, bag of feta cheese and a box of garlic toast the PIC discarded at time of inspection. Discussed with PIC to address the leak in the walk-in causing the icicles and to not store food under the frozen liquid until it is fixed. Corrective Action: Correct prior to next routine inspection.

Observed in kitchen a sheet tray holding food debris under the fryer. Per PIC that tray is switched out weekly. Also observed dried food buildup on the fryer and shelving unit next to the fryer in the kitchen. Observed broken glass, plastic, food debris and dark organic matter buildup under all the equipment in the bar. Discussed with PIC physical facility must be cleaned frequently enough to prevent accumulation of dirt, organic matter, and food debris to prevent pests. Observed missing/cracked and hanging caulking of the air ventilation system over the fryer, grill and flat top area. Observed very minimal hanging caulking above the pizza station ventilation. Discussed with PIC that all ventilation joints that are not removable to be properly cleaned shall be sealed with temperature appropriate caulking to prevent grease buildup and dripping. OC re-inspection was conducted with the owners, Antwon and Ty J. Pest control has been addressed and the establishment will continue with the pest monitoring. Permit is approved.A minor remodel permit will be created to address all leftover items.Please address the following items prior to the next inspection:1. Provide a hand sink inside the food prep room. Sink must have hot and cold water and side splashguards. The sink can discharge into the floor sink of the food prep sink.2. Provide minimum 1” air gap from the finished floor or flood rim for all drainpipes from 3-comp sink, prep/ dump sink, drain trough, ice machine, ice bins, etc.3. Certify the soda carbonator backflow preventer and provide report.4. Replace carpet floor and popcorn ceiling in the dry storage room. Ceiling and floor material must be durable, smooth, and easily cleaned material.5. Provide several coats of marine grade sealant on the surface of the brick walls in the bar.6. Provide washable ceiling tiles inside the bar. Ceiling tiles must be light color in work zones. Reviewed other ceiling finishes options for the bar.7. Remove the ceiling fans inside the bar. Bladeless fans can be used if desired.8. Provide self-closer or self-closing hinges on the deliver door.9. Repair / replace the fan in the air curtain it is rattling.10. The backflow preventer for the soda carbonator and dishwashing machine had not been service/ replace and there are signs of leaking on the units. Core-6-201.11, C: Floors, Walls and Ceilings-Cleanability __ Noted the floor in the bar is missing grout between tiles, areas around the floor sink in the bar is at aggregate level, there is a hole at the floor sink of the bar's 3-comp sink. Fill missing grout. Grout and floor must be at the same level. Concrete filler must be smooth and non-absorbent. Provide several coats of marine grade sealant on the brick walls inside the bar. Provide FRP boards behind the wet wall of the bar (where the 3-comp sink and the jockey boxes are). Provide several coats of semi-gloss paint on the substrate of the countertop. Core-6-201.12, C: Floors, Walls, and Ceilings, Utility Lines__ Please provide missing cleanout covers on walls where missing. Remove old data lines and ensure all electrical / data wiring must be enclosed inside conduits. Please remove the carpet and provide smooth, durable, non- absorbent floor surface material.
