Observed some organic matter on the inside of a soda fountain nozzle. Discussed with person in charge to clean the soda fountain nozzles on a more frequent basis to prevent the accumulation of organic matter inside the soda fountain nozzles. The person in charge removed the soda fountain nozzle in question and cleaned the nozzles and soda fountain. All food-contact surfaces must be cleaned to sight and touch. Corrective Action: Corrected at time of inspection.

Final Construction inspection conducted with site superintendent and operators. Construction completed and approved.Inspection report emailed to Jamil J.Please address the following prior to next inspection:1. Repair the ceiling above the ice machine so that it is smooth easy to clean.2. Label the 3-compartment sink in reverse order so that sump pump installed is over the dirty dish area or move 3-compartment sink a foot over to the left side.3. Continue to seal penetration from riser, soda syrup lines, and hole adjacent to the toilet in the women's restroom.4. Cutback and adjust the 3-compartment sink so that it maintains 2-inch air gap with floor sink. Adjust the drain so that it reduces water splash out of floor sink. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Provide 30-gallon water heater in lieu of current unit.2. Ensure hot water is provided within 45-seconds to all hand sinks and measures between 100°F -110°F.3. Provide an NSF 3-compartment sink with drainboards indirectly connected to a floor sink.4. Provide an NSF commercial grade hand sink with metal splashguards for the front counter area and adjacent to 3-compartment sink.5. Provide a smooth easy to clean surface above the 3-compartment sink (dry wall with semi-gloss paint was mentioned as solution in lieu of scrim sheeting).6. Certify all testable backflow preventers and provide copies of the backflow preventer test certifications.7. Provide weather stripping for the exterior doors that allow light to shine through.8. Provide a uni-strut for the drain line at the front counter to maintain air gap with floor sink.9. Provide several coats of semi-gloss paint for wooden support of water heater.10.
