The establishment was open and operating at time of inspection. Please address the listed re-inspection items and the below violation prior to the next OC re-inspection. Prep Sink MCEHC §8.1.8g (1), (a) ------- Provide a separated, single compartment prep sink for rinsing produce and thawing raw meats. Ensure hot and cold water is provided. Indirectly plumb the sink to the floor sink and provide a 1" minimum air gap. Reviewed the major foodborne illnesses and their related symptoms. MCESD inspection report was delivered via email.

To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. PRIORITY VIOLATION-5-205.15 (A), P: System Maintained in Good Repair; According to Law 1. A separate stainless-steel NSF approved food preparation sink is required for the proper rinsing/thawing of food items. Drain lines must be indirectly plumbed with an approved minimum 1-inch air gap. 2. Provide an approved handwashing sink with metal splashguards adjacent to the 3-compartment sink. 3. Repair the atmospheric vacuum breaker for the mop sink faucet repaired (leaks when used). 4. Provide an ASSE 1015 or ASSE 1013 RPZ backflow preventer upstream of steam injected ovens (the waterline connection is under the 3-comp sink). 5. Certify all testable backflow preventers (ASSE 1013 soda and ASSE 1015 oven) and provide copies of the backflow preventer test certifications. 6. Provide a rigid drain line for the water filter relief drain (PEX tubing to rigid drain). Ensure it drains to the floor sink (as well as soda cabinet drains). 7. Provide uni-struts for all drain lines so that they maintain 1-inch air gap with the floor sink. 8. Ensure to clean floor sinks and restore there smooth easy to clean finish (organic growth covering them). Correct By: Correct Prior To Reinspection
Core-6-501.11, C: Repairing-Premises, Structures, Attachments, and Fixtures-Methods 1. Seal all penetrations into the wall and ceilings (syrup lines, electrical lines, inside walk-in). 2. Provide 10-foot candles of lighting for the walk-in cooler. 3. Seal the gap between the wall and 3-compartment sink (you may use backerod). 4. Remove unused syrup lines from soda system. 5. Reattach the BIB system to the wall where it has fallen. 6. Deep clean underneath equipment as needed. Correct By: Correct Prior To Reinspection
