Correct the following items prior to the next inspection: 1) Provide a metal splash guard on the right side of the front hand-sink. 2) Provide washable ceiling tiles above the soda tower. 3) Provide a minimum 1-inch air gap off the RPZ funnel drain located under the soda tower. 4) Plumbing drain line under the 3-comp. sink is wet. Repair and maintain. No legal action will result from this inspection.

To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. If you select payment via US Mail, for further instructions please refer to the printed invoice. No operating permit is valid, until the permit fee is received by the Department. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes.No county legal action will result from the inspection. PRIORITY VIOLATION-5-205.15 (A), P: System Maintained in Good Repair; According to Law 1. A separate stainless-steel NSF approved food preparation sink is required for the proper rinsing/thawing of food items. Drain lines must be indirectly plumbed with an approved minimum 1-inch air gap. 2. Certify all testable backflow preventers (ASSE 1013 soda carbonator) and provide copies of the backflow preventer test certifications. 3. Provide a rigid drain line for the water filter relief drain (PEX tubing to rigid drain). Ensure it drains to the mop sink (as well as soda cabinet drains). 4. Provide uni-struts for all drain lines so that they maintain 1-inch air gap with the floor sink. 5. Ensure to clean floor sinks and restore, so there a smooth easy to clean finish (organic growth covering them). 6. Repair leaking condensate drain under 3-comp sink. Replace the acoustic ceiling tiles above the ice and soda fountain dispenser with smooth easy to clean ceiling tiles. 3. Seal all penetrations into the wall and ceilings (syrup lines, electrical lines, inside walk-in). 4. Clean the organic material at 3-comp sink and reseal. 5. Provide cleanout cover where missing.
