To complete the issuance of this permit and begin operation, the applicant must first submit payment of the permit fee to the Department’s Business Services Office. Per Chapter I, Regulation 4, of the Maricopa County Environmental Health Code, no person shall conduct an operation or an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit card can be made in person, by US Mail, by or phone at 602-506-6824 Option 2. We accept cash, check, money order, credit/debit. It is the responsibility of the permit holder/owner to notify MCESD if there are contact/ billing address changes. All permit fees are nonrefundable.On July 1st, 2023, the Environmental Services Department started emailing permit card(s) and permit invoice(s) to permittees. Provide an NSF approved, 3-compartment sink that is capable of fitting your largest sheet pans so that they can be submerged at least half-way.2. Provide FRP on the wall behind, underneath to the floor and around the new corner location where the mop sink will be moved to. Use FRP Adhesive and trim for the edges. Do not use nails or screws.3. Provide cove base at all floor/wall junctures where missing. Cove base should be installed on the surface of any new FRP sheets installed.4. All sinks must have separate drain separate drain pipes that empty into the floor sink with minimum 1 in. air-gaps between the ends of the drain pipes and the flood rim of the floor sink.5. Public access to the only restroom in the ware washing area will not be permitted.6. Arrange the new dough prep area so that the front hand sink is visible to the work station area.7. Do not run extension cords on the floor of the kitchen areas.8. Plug and seal the unused pipes in the floor beneath the restroom hand sink.9. Ensure that the water heater is 50 gallons capacity.
