Observed the mop sink faucet detached and able to be pulled out from the wall, as well as a small leak from the faucet. Informed PIC that the faucet needs to be resealed to the wall and brought back into good repair. All plumbing must be maintained in good repair at all times. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Corrected at time of inspection.

Observed cardboard being used as a backsplash behind the front prep counter to the left of the cashier stand. The cardboard has dark grease and pizza sauce stains. Informed PIC that cardboard cannot be used as a facilities material due to its porous and absorbent structure making it non-cleanable. PIC stated that they will discard the cardboard and discontinue use of it for backsplash purposes. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.
