Observed Standing water in floor drain under 3 - bay sink, and in Mop Sink. PIC stated the issue has been ongoing and due to a plumbing issue in the alley outside of the building. Observed in the alley an old grease trap with water coming out of it and into the alley. A permit holder shall immediately discontinue operations and notify the regulatory authority if an imminent health hazard may exist because of an emergency such as a fire, flood, extended interruption of electrical or water service, sewage backup, misuse of poisonous or toxic materials, onset of an apparent foodborne illness outbreak, gross insanitary occurrence or condition, or other circumstance that may endanger public health. Prior to lifting the suspension, all sewage must be properly conveyed in the facility and all water in the alley must be properly cleaned up. Corrective Action: Correct at once.

Inspector's notes

Observed sandwich cold table with an ambient temperature of 59*F measured with inspector min/max thermometer. Cooler contained TCS items including dressed coleslaw, sliced/diced tomatoes, chopped lettuce, sliced ham, sliced turkey, sliced roast beef, shredded turkey, diced chicken, tuna salad, mayonnaise, and sliced provolone cheese. The person in charge stated that the products were stocked in cooler approximately 5 hours prior to inspection. There was no record of temperatures for the cooler available. With inspector probe thermometer the products measured between 59*F -65*F. The person in charge voluntarily discarded the product at time of inspection and set up ice bath for new product. The person in charge stated that a work order would be placed. Discussed with the person in charge that all coolers should be maintained at 41*F or below when cold holding TCS items. Corrective Action: Corrected at time of inspection.

Observed rodent droppings on the booth and under the tables in the side dining area. Observed rodent droppings, chew marks on boxes, and debris on and under the dry shelving in the back of the kitchen. In the pizza prep area, observed rodent droppings on the floor behind the coolers, in the cabinets, inside and under the 3 compartment sink, and on and under the extra signage. Reviewed the most recent pest control report which said they had caught two mice. Per manager, it is a 2 month treatment plan and they are still working to complete this process.
