Inspector's notes
Observed drainpipe for the dish machine entering below the flood rim of the floor drain. Informed PIC that there needs to be an air gap of at least one-inch between the outlet of the pipe and the top of the floor drain. PIC input work over at time of inspection. All plumbing must be maintained in good repair at all times. Must repair all plumbing to a state of good repair prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. PRIORITY VIOLATION-5-205.12 (A) - Priority: Prohibiting a Cross Connection; Drinking Lines Addl Notes: Observed a three-way water valve with shut off attached to the mop sink and a chemical dispenser. Informed PIC that shut-off valves, including chemical dispensers are not approved for continuous installation downstream from an AVB. Observed water spigot open near sink. Attached chemical dispenser to the spigot to give the dispenser a dedicated water supply. PIC removed the three-way valve and chemical dispenser at time of inspection. Shut-offs, including chemical dispensers and sprayer nozzles, are not approved for continuous installation downstream from atmospheric vacuum breakers. Corrective Action: Corrected at time of inspection.

Observed opened Mozzarella cheese having a date marking of 9/27. Also observed carnitas without a date marking. Person in charge states the carnitas was opened 3 days prior to the inspection and properly labeled it at the time of inspection. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). All RTE/TCS foods past 7 days of preparation were discarded by PIC at the time of inspection. Corrective Action: Corrected at time of inspection. 2ND REPEAT VIOLATION.
