Observed broken and missing floor tiles throughout the kitchen and establishment. Observed a large portion of the floor tiles missing in the ware washing area. Observed the paneling peeling off of the wall in the handwashing doorway, leaving a gaping area for dirt and debris to stick. Observed multiple holes in the walls near the floor. Informed PIC that these areas need to be repaired and done so in a way that the walls, floors, and ceilings are smooth and easily cleanable. Discussed surveying the establishment routinely to look for items that may need to be repaired with PIC. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Physical facility must be maintained in good repair at all times.
