Observed cove base throughout the main kitchen is disrepair with the original cove base removed. Observed some areas with a 3/8" radius curve present but not properly sealed or with sections of wall immediately above the curve cracked or in disrepair. Physical facility must be maintained in good repair at all times. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection. 6-201.11 - Core: Floors, Walls and Ceilings-Cleanability Addl Notes: Observed multiple wall holes throughout the establishment where section of walls were removed to repair plumbing or where cleanout covers have been removed. All floors, walls, and ceilings must be maintained and repaired such that it is smooth and easily cleanable. Repair physical facility prior to next routine inspection. Advised PIC to restore wall and utilize cleanout covers to provide access to plumbing, as needed. Corrective Action: Correct prior to next routine inspection.

In the walk-in cooler observed a container of coleslaw with a date mark of prepped 01/25 and a container of meatballs with a date mark of prepped 03/03. RTE/TCS foods requiring date marking must be date marked within 24 hours and sold or discarded after 7 days (including the day it was made as day 1). All RTE/TCS foods past 7 days of preparation were discarded by person in charge at the time of inspection. Corrective Action: Corrected at time of inspection.
