Observed ice formation on the floor and in buckets underneath the condensation line in the freezer. Condensation line currently has tape wrapped around it to prevent leakage. Repair in a permanent manner so that all condensation is drained out of the freezer and into its designated floor sink. Corrective Action: Correct prior to next routine inspection.

Person in Charge (PIC) stated that pest control service came out to the establishment in the past month. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Must eliminate current pest control problem and obtain pest control from a certified and licensed pest control specialist by re-inspection. Corrective Action: Correct prior to re-inspection.
