Predefined Comment: Discussed the importance of proper hand washing and when hand washing is required with the person-in-charge (PIC). Hands must be washed at any time contamination occurs and when changing tasks. Hands must be washed after touching bare human body parts other than clean hands and clean, exposed portions of arms. Predefined Comment: Employee properly washed hands at handwash sink at time of inspection. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-5-205.12 (A), P: Prohibiting a Cross Connection Observed mop sink faucet equipped with an atmospheric vacuum breaker (AVB) with a water supply line hose attached and directly feeding to a chemical dispenser. Observed system under pressure at time of inspection. Under such installations the AVB device will fail under constant pressure conditions and a potential cross-connection hazard is made. The water supply line from the mop sink was disconnected from the chemical dispenser at time of inspection by the person in charge.
