Observed 2/3 storm drains on back dock to be clogged and breeding mosquitos. Mosquito larvae in both drains. Person in charge (PIC) poured a bottle of mineral oil on one drain and chlorine in the other drain. Follow-up in 10 days. Establishment must be maintained free of pests and all pest control measures must be conducted by a licensed pest control specialist. Inspector's notes

Establishment has an approved HACCP plan and variance for wet shellfish tanks. One of the tanks was holding live oysters in water at 43'F and the other tank had water at 45'F. Ice was added to the tank to cool down the water. After about 45 minutes, water temperature was 39'F. Water temp was taken earlier in the morning and was in compliance at that time. In addition, test results are only conducted monthly, not weekly, at a 3rd party lab,. The approved HACCP plan states that the test is performed weekly. A follow-up meeting will be held to determine if the 3rd party lab tests must be conducted weekly or monthly. Corrective Action: Correct prior to re-inspection.
