Observed the meat band saw stored as cleaned with visible food debris on the blade. PIC had the machine washed, rinsed, and sanitized in place (CIP) at the time of inspection. Corrective Action: Corrected at time of inspection. *Second consecutive repeat violation* PRIORITY VIOLATION-4-702.11 - Priority: Cleaned and Sanitized Before Use After Cleaning. Observed various cooling foods stored on the ground blocking access to the only hand sink in the kitchen area. Observed raw chicken breasts stored on the counter in the meat market not under refrigeration. PIC stated they had been left out to be cleaned at a later time. All TCS foods improperly cold held were relocated to alternative functioning refrigeration unit to rapidly cool to 41°F at time of inspection. Except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. TCS foods not actively undergoing preparation must be maintained at 41*F or below. Corrective Action: Corrected at time of inspection.

Observed three unpackaged fish heads at the bottom of the customer self-service grab and go freezer. The freezer contained packaged raw seafood products. Mentioned to PIC that fish heads were observed and PIC discarded them immediately voluntarily. PIC mentioned fish heads possibly fell out of packaging. Consumer self-service of raw animal proteins is not approved to prevent cross contamination. All consumer self-service of raw animal proteins was discontinued at time of inspection.
