Per employee, lettuce, tomatoes and thousand island were all placed in prep table less than an hour to inspection and were moved to the walk-in to rapid cool. Per employee, onions in buttermilk, chipotle ranch, & ranch dressing were in the prep table from the day prior & were embargoed at time of inspection. Observed ambient temperature of prep table at 44.6*F at time of inspection. Establishment has another working prep table and has agreed to discontinue use of the broken prep table until it can maintain ready-to-eat (RTE) & time and temperature controlled foods for safety (TCS) at 41*F or below.
