Inspector's notes

Observed on menu at the register to have no posted consumer advisory or disclosure method. Discussed with PIC offering and serving undercooked proteins requires an approved consumer advisory at all points of sale; and must contain an approved disclosure method and reminder statement. Items requiring the consumer advisory: all nigiri, all sashimi, fire tuna roll, fire salmon rolls, rainbow roll, spicy rainbow roll, salmon sky, Boston roll, shrimp lover roll, sunny roll, volcano roll, Mexican roll and spicy tuna roll. Must provide approved consumer advisory at all points of sale prior to re-inspection. Correct prior to re-inspection 8-10-26 2nd Consecutive Violation

Inspector's notes

Observed in the reach in refrigerator and also the prep table in front of the wok station to have several items that have brown/black mushy spots, black fuzzy organic matter or white fuzzy organic matter. Discussed with person in charge (PIC) all food must be in good condition, safe, and unadulterated. All adulterated food was discarded at time of inspection. The discarded items are peppers, green onion, zucchini, cabbage, micro greens, and precooked ramen noodless. Corrective Action: Corrected at time of inspection.

Inspector's notes

Observed TCS items in the reach-in unit to be at temperatures of 46*F to 55*F measured with a probing thermometer. The reach-in unit had an internal ambient temperature of 51*F measured with a min/max thermometer. Per PIC all items were in the unit from the previous day. Discussed with PIC except during preparation, cooking, cooling or when time is used as a control all TCS foods must be maintained at 41°F or below. Eggs not treated to destroy Salmonellae can be stored in equipment that maintains an ambient air temperature of 45°F or below. All TCS foods improperly cold held were discarded by PIC at time of inspection. The discarded items were raw shelled eggs, shredded crab meat and rehydrated wood ear mushrooms. Several packages the shredded crab meat were at 43*F or less and were relocated to the freezer at the time of inspection. Corrective Action: Corrected at time of inspection.

Observed the reach-in unit had an internal ambient temperature of 51*F measured with a min/max thermometer. Observed the unit holds only sheet trays as shelves and the fan was blocked by boxes of mayonnaise. Also observed the sheet trays were solid and did not allow for any airflow. Discussed with PIC Cold holding equipment must have the capacity to maintain TCS foods at 41°F or below. Must repair cold holding equipment such that it maintains TCS foods at 41°F or below. Corrective Action: Correct prior to re-inspection 7-9-26
